sidewalk repairs

Does anyone know of a good contractor who does sidewalk repairs? In addition, if anyone knows had to go about eliminating a sidewalk violation with the Department of Transportation that would be helpful too. For instance, do you need to show them some sort of contract or picture of the sidewalk once it is repaired?
